CCDW Permit Address Change

I'm looking to have my address updated on my carry license. What do I need to do and what's the fee? Can I still carry when my address doesn't match my drivers license? I found this form online on the Kentucky State Police website. Help! Lol

I went to my county sheriffs office. Filled out a form and waited. Took about a month and I went and picked up my updated permit. $15 or $20 if I remember correctly. And sheriff said yes I cOuld carry with my existing permit while my new one was being processed. Simple enough.

Actually I think the change was free now that I read that form again. The fee was when I changed my drivers license.
