Been having this issue since getting the Tac Sol upper. At least once a mag a round will go nose up into the top of the chamber.

I don't remember ever having this issue with the factory upper. This one fits looser than the factory, not sure if that could be part of it. Need to take the old upper with me next time I go and try both to be sure

YouTube how to tighten these up. There are lots of videos. I think that was the problem with mine before I got it. I'm telling you, it needs to visit Coach for a little while...haha

I had to "fit" a barrel for a friend, just bang the engaging lug till it fits tight. Polishing and contouring of the ramp is standard affair.

I'm not sure it's the ramp that's the problem. The bolt is ending up over the cartridge

Took it back to the range with me today when I went to shoot the VP9. Only shot a few mags through it but confirmed that if I held the upper down tight to the frame it would work fine. If I pushed the upper up away from the frame it would fail.

Came home and poured out a PBR (not drinking right now so there were no empty cans) and started cutting it up. Ended up with a tiny piece folded over so there are three layers. Put a little bend in it and epoxied it onto the bottom of the slot in the upper that the lug on the front of the lower engages. Took a couple of tries to get it right. Let it sit for an hour and now it's plenty tight when assembled. Just need to go shoot it again now

I used a small brass hammer and tapped the lug till I got a tight fit. There is a YT video out there that shows how.

I wanted to avoid modifying the frame so that the original upper could still be used on it. Don't know why, just wanted to keep it able to
